In Euroland it's seems to be a gesture to show you appreciated somebody doing a great job. In my experience that's somewhat rare as you just get a standard level of good service and the tips are very small accordingly. Here in the Thunderdome you have service that can be amazing all the way down to "waitress annoyed that you asked for a fork". (French restaurants had superb service of course)

I tip good waiters 20%, and slide the scale from there based on performance. If it gets to the point that I'd not leave a tip, I've already talked to the manager and gotten someone else to serve us, and that has only ever happened once. I find that if you're not a dick to them, they are usually pretty nice back.
